ADDITIONAL TERMS AND CONDITIONS FOR APPLECARE+ MONTHLY RECURRING CHARGE OPTION

AppleCare+ Protection Plan - If you purchased the AppleCare+ Protection Plan (Plan) pursuant to the option to pay by a monthly recurring charge, you hereby appoint NWMC as your agent and authorize NWMC to act on your behalf to cancel or modify your AppleCare+ contract with Apple upon (1) your cancellation of the Plan; (2) non-payment of your applicable monthly recurring charge; or, (3) termination of service pursuant to the terms of this agreement.  

If you purchased the Plan pursuant to the option to pay by a monthly recurring charge you cancel the Plan within thirty (30) days days of the purchase of your iPhone, you will receive a refund for the first month’s charges of the Plan, and no further charges will be assessed for that service.  For cancellations made after thirty (30) days of your purchase of the Plan, no further monthly charges will be assessed for the service after the date of cancellation, but no refund will be issued for any monthly charges paid for the Plan up to that point, and you will be assessed a cancellation fee of twenty-five dollars ($25).
